Fourthly: Download the Genealogical form (your family history), which is presented below, by clicking here  Now this is very important for WOMEN: Please use your "Maiden" name in place of your "married" name to complete this genealogical form.

Finally: After you have download the Instructions and the two forms, complete them and phone our (Elder) Membership Director, Grace Person, at 604-703-1481.
